A 62 years male who had Ca esophagus - got esophageal resection and gastric pull up - presented with severe pneumonia with respiratory failure 6 months back. He underwent CT chest showing a fistulous tract between the pulled-up stomach and trachea confirmed later with UGIE and bronchoscopy. Subsequently ASD device II was used to close the tract with success and patient improved and got discharged. Other than cough he was doing fine and was undergoing immunotherapy for his recurrence of tumor. He presented now with worsening cough and vomiting and expectorating bilious sputum. Repeat bronchoscopy showing fistula reappearance from the side of the device. Need an expert opinion regarding further line of treatment. My concern is about removing the ASD device?
