50-year-old female, ex smoker who presents with dyspnea, chest tightness, cough and right pleural effusion. She underwent thoracentesis first, the fluid was an exudate, cytology was negative, cell count showed 68% lymph. Thoracoscopy with pleural biopsy came back diagnostic for metastatic adenocarcinoma consistent with Mullerian primary. PET CT was only remarkable for mild linear uptake at the right posterior pleural surface. Pelvic MRI was negative.
